AACC Athletic Participation Medical Requirements

*All forms must be thoroughly completed and on file before student-athletes may try out or participate on a sports team, including all preseason and out of season activities.*

Remember:

  • Your physician must sign the physician's clearance and exam forms. You must use the AACC physical forms.
  • Answer all medical history questions accurately and thoroughly to ensure that you will be given the BEST possible care in the event of an injury
  • Accurate insurance information is needed to ensure treatment in the event of medical transport to a hospital
  • You may NOT try out or participate in any off-season or preseason activities until all medical forms are completed and on file!
  • All physicals are good for 13 months from date of exam
  • All physicals must be on the AACC physical form (No Exceptions)

Returning Student Athletes (click here)

Prospective/New Athletes

  1. Open aacc2.atsusers.com in a browser

  2. Enter the following for the Athlete ID and password:

    1. Athlete ID: NEW

    2. Password: NEW

    3. (You will be prompted to create your own ID and password on the next screen)

  3. Enter your personal information and click “Save”

  4. Continuing entering information through the different tabs at the top of the page and click save on each tab. All tabs are mandatory.

    1. General

    2. Insurance

    3. Contact

    4. Note: a red dialog box indicating remaining tabs will pop up after completing each tab

  5. Once these tabs are complete, additional tabs will appear (Surgeries, Screen/Tests, Paperwork, Forms, eFiles)

  6. Click on Forms tab

    1. Using the Form Name dropdown menu, select a form on the list and click the white “New” button

    2. Complete this form, electronically sign it, and click the blue “Save” button at the bottom of the page

      1. Note: on the Concussion SCA Form and the Liability Form, you must click “Yes” to acknowledge that you read and understand each section  

    3. Continue until all required forms are complete

    4. The list of completed forms will populate at the top of the page

  7. Click on the eFiles tab

    1. Download and print the Physical Form

    2. Bring this form to your doctor to complete and sign

    3. Upload the completed and signed form using the blue “Upload Document” button (max document size 1MB)

  8. All athlete information must be complete and a physical examination completed within the last 13 months in order to try out or participate in any team activities.

Returning Athletes

  1. Open aacc2.atsusers.com in a browser

  2. Log in with previously created ID and password.

    1. Forgot your password? Click “Forgot Password” to reset it

    2. Forgot your Athlete ID? Click “Forgot Athlete ID” to retrieve this information

  3. Update all information across all tabs

  4. Click on Forms tab

    1. You must complete new forms each year

    2. Using the Form Name dropdown menu, select a form on the list and click the white “New” button

    3. Complete this form, electronically sign it, and click the blue “Save” button at the bottom of the page
